Medical reimbursement claim documents:
a practical checklist.
Which documents to keep and check before submitting a CGHS, Madhya Pradesh state government or private health insurance reimbursement claim.
The short
answer.
Most medical reimbursement claims need the claim form, original hospital bills and payment receipts, the discharge summary, prescriptions, test reports, pharmacy bills, your beneficiary or policy details, and bank details for payment. Some schemes also need an essentiality or emergency certificate.
The exact list depends on your scheme or insurer, the type of treatment and whether the hospital was empanelled or in-network. Always check the current requirements with your office, CGHS or your insurer or TPA before submitting.
CHECKLIST
Documents to
gather.
Keep originals safe and make copies before you hand anything over.
- Claim form. The form your scheme, office or insurer specifies, filled in and signed.
- Hospital bills and receipts. Final itemised bill and proof of every payment made.
- Discharge summary. Showing diagnosis, treatment and admission and discharge dates.
- Prescriptions. Doctor's prescriptions and treatment advice, including for medicines bought outside.
- Test and investigation reports. Reports matching the bills submitted.
- Pharmacy bills. Each one matched to a prescription.
- Beneficiary or policy details. CGHS card, employee or pensioner ID, or insurance policy and TPA card.
- Certificates, where required. For example an essentiality or emergency certificate, or a referral or permission letter.
- Bank details. A cancelled cheque or account details for payment.
- Identity proof. If your insurer or office asks for it.
Common reasons
claims are returned.
- A bill or receipt is missing, or is a photocopy where an original is required.
- Pharmacy bills don't match a prescription.
- Patient names, dates or amounts differ between documents.
- A required certificate or signature is missing.
- The claim is submitted after the deadline set by the scheme or policy.
Checking the set against your scheme's list before submission avoids most of these.
COMMON QUESTIONS
Good to know.
Where do I submit a CGHS or state government reimbursement claim?
Serving employees usually submit through their own office or department. CGHS pensioners usually submit through their CGHS wellness centre or city office. Check the current procedure for your case.
Is the checklist the same for private health insurance?
It is similar, but insurers and TPAs set their own claim forms, deadlines and document lists. Check your policy documents or ask your TPA.
